Referral

New patients can receive a referral from their primary care provider, dentist, or allied health practitioner. Once your referral is received, our Patient Onboarding Department will triage it, and you’ll be scheduled for a consultation.

Insurance & Payment

  • Health Card: Required at every appointment.
  • Medication List: Bring a recently updated list of all current medications, along with any past medications. This information helps us determine additional treatment options.
  • Medical Documents: Your referring physician typically sends necessary medical documentation, but please bring any additional reports that may be relevant.

What to Bring

Your health card. Please note, you are expected to bring your health card to every appointment at NeuPath Centre For Pain and Spine.

A recently updated list of all the medications that you are currently taking from your pharmacy. As well, a list of medications that you have used in the past can be obtained from your pharmacist at your request. This list of medications can be extremely helpful in determining additional treatment options at the time of assessment.

Your referring physician or medical specialist will normally forward the required documentation and medical history required by our clinic. If you have any additional reports that your doctor may not have, please bring those to your first appointment.
